Let us help your patient

Communication, inclusion and empathy are cornerstones of our referral and support network. Road Home is dedicated to a highly-collaborative process that starts with your phone call to an in-take coordinator at (312) 942-8387. Your patient will work with an admissions specialist to assess whether your patient is a good fit for our program.

Multiple treatment models

We don’t have a one-size-fits-all model of intervention because each veteran is unique. At the Road Home Program, we offer two models of therapy that fit most veterans’ needs.

Outpatient Care

For some, we recommend a more traditional outpatient model as a better fit for veterans and their families. We develop a treatment plan for mind, body and spirit that focuses on immediate symptoms reduction, long-term mental health recovery and permanent lifestyle changes for the veteran. Over time, veterans and their families leave Road Home with the tools they need to take control of their health, their jobs, their relationships and their family lives.

Intensive Outpatient Program

Veterans in need of more concentrated care for PTSD or MST can apply for admission to our two-week Intensive Outpatient Program (IOP). Participation is based on a pre-admissions interview with the veteran that determines motivational and logistical readiness, call (312) 942-8387.
