• Your job: If you are employed, our team can help you determine if you need short-term disability and coordinate the required paperwork. Please let your intake clinician know.
• Transportation: If coming in from out of town, we will coordinate travel arrangements including airfare, Amtrak train or bus fare. In addition, we cover transportation to and from the airport, bus or Amtrak train station. If driving, we provide mileage reimbursement via gift cards.
• Food: We provide meals for you during your scheduled programming. Please advise us of any food allergies prior to arrival. Medical documentation must be provided for any food allergies.
• Limitations: Please let us know if you have a disability, need an assistive device and/or if you plan to bring a service animal with you.

• Prescription medications (bring adequate supplies for the duration of your stay)

• Walkers, crutches, canes, hearing aids or magnifying glasses

Identification

• A copy of your DD 214
• A photo ID
• Your insurance card (if you have insurance)
• Names and phone numbers of family members or friends to contact in an emergency

Personal

• Toiletries (e.g., shampoo, toothbrush, toothpaste, soap)
• Comfortable clothes, including daytime wear and pajamas (check the weather in Chicago and remember to pack according to the season)
• Exercise clothes and athletic shoes

What NOT to bring

• Weapons of any kind
• Alcohol
• Any non-prescription (legal or illicit) substances or medication

You will be staying at the Illinois Medical District Guest House, which is a furnished, apartment-style suite including a kitchenette and on-site laundry facility.

The IMD Guest House is centrally located on the University of Illinois Chicago campus. Rush shuttle service will take you to and from the IOP every day for programming.
