BRIDGE RESOURCES

BRIDGE is a self-guided directory of community resources that may be helpful to veterans, service members, military families, caregivers, providers, and community partners.

Resources are identified according to the Road Home Program’s relationship with each organization:

RHP Community Partner — An organization with an established partnership or referral relationship with RHP
Vetted Resource — A resource reviewed and recommended by RHP clinical staff
Community Resource — A publicly available resource that families can explore independently

Important: Inclusion in this guide does not guarantee eligibility, availability, quality, or suitability for an individual or family. Unless identified as an RHP Community Partner, the Road Home Program at Rush University Medical Center does not have a direct affiliation or referral relationship with the organization. RHP staff cannot answer detailed questions about the eligibility, availability, costs, or services of external organizations. Please contact each organization directly for current information.

For questions about Road Home Program services, submit a Get Care form or call (312) 942-8387 (VETS).
